The Advanced Accident Investigation course aims to equip managers, supervisors, and safety advisors with the skills to conduct successful accident investigations, determine root causes, provide long-term solutions, and perform full cost analysis. This advanced course covers accident understanding, prevention techniques, investigation principles, and cost assessment. Course Aims:
To provide managers, supervisors and safety advisors with the skills and knowledge to undertaken successful accident investigations, interview techniques, determining route causes, providing long term solutions and full cost analysis. This course is more advanced than the basic Accident Investigation Course but no formal
pre-qualification is required.
Day 1
Introduction and Setting Course Objectives
Understanding Accidents
What is an accident? Understanding the difference between accidents and injuries. The relationship between injuries, accidents, near misses etc. Understanding the difference between hazards and risks in construction. Breakdown of the types of accidents and ill- health encountered in construction. Recognising hazard groups and their effects.
Accident and Ill-Health Reporting
Reportable accidents, notifiable dangerous occurrences and illnesses appertaining to the construction industry. Who has to submit the reports and to whom? Non-statutory notifications, lost time accidents, accident book entries near-miss recording. Benefiting from accident statistics.
Practical Exercise:Establishing a schedule of accident and incident reporting categories.
Accident Prevention Techniques
Tried and tested methods of accident prevention. Domino theory. Total loss control.
Accident Investigation
Principles of accident investigation. Taking of statements. Detailed investigation of facts and contributory factors. Powers of inspectors to take statements. Providing evidence to solicitors and the HSE.
Day 2
Accident Causation
Determining the actual causes of an accident, including ‘Multiple Cause Diagramming’.
Practical Exercise: Establish the cause of an accident using multiple cause diagramming.
Investigation Interviews
Management of interviews and techniques to be used. Interview records.
Practical Exercise: Role plays in interviewing.
Cost of Accidents
Establishing the true cost of accidents in financial terms as well as in terms of human suffering. Hidden costs of accidents. Employers liability compulsory insurance.
Practical Exercise: Establishing the cost of an accident.
Investigation Reporting and Corrective Actions
Cost/Benefit decisions for corrective actions. The structure of an accident investigation report.
Practical Exercise:Production of an investigation report.
